We are located at: 1310 N. Main Street, Suite 101, Sandwich, IL 60548

From Route 34, turn north on Main Street which will dead end at the office building parking lot.  The medical office building is on the Valley West Hospital campus, just north of the hospital building.  Close parking (including handicap parking) is readily available.  Upon entering the office building, we are the first suite on the right.
